Important Forms

To help eliminate paper notes to the main or health offices, we are asking families to submit notes electronically regarding attendance.  Instead of sending in a note, please use the following forms:

• Absence Form 24.25
• Early Pick-Up/Dismissal Forms 24.25 Please complete by noon if you need to pick your child up early. We are not able to accurately accommodate requests after noon unless it is an extreme emergency. This will be determined by the principal.

If you would like to be on the permanent pick-up list, please complete this form: Leary Permanent Pick-Up Form 24.25 

These forms will automatically go to a spreadsheet in the main office. Of course, you can also call the attendance line at 359-5471 to report your child’s absence.
